Scientific research in obstetrics and gynecology: changes in the trends over three decades

The aim of this work was to assess scientific research of master’s and doctoral theses and essays in the Department of Obstetrics and Gynecology, Zagazig University, Egypt.

Materials and methods

All master’s and doctoral theses and essays since the foundation of Department of Obstetrics and Gynecology, Zagazig University, Egypt, in 1975 till end of 2012 were reviewed.

A total of 703 theses and essays were reviewed. The important topics in the specialty of obstetrics and gynecology were covered and updated. Infertility, in vitro fertilization–embryo transfer (IVF-ET) and related techniques, and polycystic ovarian disease were the most common gynecologic topics (27.2%), followed by gynecologic oncology (18.5%). Preeclampsia was the most common obstetrics topic (18.8%), followed by issues of high-risk pregnancy, fetal growth restriction, and fetal well-being (11.6%). The number of researches that allow the candidates to learn skills was 183 and it was increased from 4.4% of all research in the period 1979–1988 to 33.2% in period 1989–2000 then slightly decreased to 31.2% in period 2001–2012. Ultrasonography was on the top and was present in 99 out of 183 (54.1%) followed by laparoscopy (30, 16.4%), hysteroscopy (25, 13.7%), IVF-ET and related techniques (16, 8.7%) and colposcopy (13, 7.1%) researches. Multi-disciplinary research was decreased by 61.7% in the period 2001–2012. Researches in academic fields were abandoned and in some clinically important areas like preeclampsia were decreased.

Scientific research of master’s and doctoral theses and essays was comprehensive, updated, and had some autonomy independent of plans. Research which enable the candidate to learn skills were increased on the expense of academic, clinical and multidisciplinary research. It could be recommended that plans for scientific research should be flexible and should leave a space for local departmental views. Proper training of residents during their rotation in these subspecialties may help to revive the lost interest in clinically important areas.

Introduction

Active participation or submission of a scientific research is an essential step in residency or scholarship programs. 1 Studies have noted that research improves analytical and communication skills of the candidates as well as lifelong learning. 2 It was also observed that residents interested in research and publications have greater academic success. 3 , 4

According to the rules of Supreme Council of Universities, Egypt, submitting a scientific research in the form of a thesis or an essay is one of the requirements to gain a master’s or doctoral degree. Selection of the topic of the research is related to many factors such as availability of cases, instruments, subspecialty, and personal view of the supervising professor. The interest of the candidate is also important. In 2013, discussions started within the faculty to develop a plan for scientific research for the next 5 years. There was a need to review all research to get an impression about previous work. The aim of this study was to assess scientific research of theses and essays accepted in the Department of Obstetrics and Gynecology, Zagazig University, since its foundation in 1975 till end of 2012.

This retrospective observational study was conducted at the Department of Obstetrics and Gynecology, Faculty of Medicine, Zagazig University, Egypt, from January to March 2013.

All accepted theses and essays since the foundation of the department in 1975 till end of 2012 were reviewed. The review included title of research, year of acceptance, and supervision from another department. Methodology and skills that the candidate should learn, such as ultrasonography, endoscopy, in vitro fertilization–embryo transfer (IVF-ET), and colposcopy, were checked. Assessment also included the influence of establishment of new specialized units on scientific research. For the sake of convenience, the term “research” will be used to refer to essays and theses in this study.

The Department of Obstetrics and Gynecology, Faculty of Medicine, Zagazig University, was founded in 1975 as a small department under the supervision of Ain-Shams University (the mother university). Then the department became separate and also grew larger. There was an increase in the number of beds, the number of undergraduate and postgraduate candidates, staff, and subsequently the number of research activities. The formation of specialized units of laparoscopy, hysteroscopy, IVF-ET, and ultrasound started in the first half of the 1990s. Colposcopy was present since 1980. A Gynecologic Oncology Unit was founded officially in the first half of 2000s. We have no gynecologic urology unit, although there are some preparations for it.

One of the requirements for a master’s degree is to submit either a thesis or an essay. An essay is review article. For a doctoral degree, a thesis should be submitted. Each thesis or essay should have a supervising committee comprising a professor, assistant professor or lecturer(s), a system similar to mentorship. 5 If part of the work will be done in another department, a supervisor from that department will also be part of the committee. According to rules in the university, supervision from another department is recommended, but is not obligatory. Candidates are encouraged to gain training and supervision from national and international experts in the field of the research.

The study was approved by the Obstetrics and Gynecology Council of the faculty. Data were represented as mean and standard deviation.

A total of 703 theses and essays were reviewed. There were 575 (81.8%) and 128 (18.2%) theses and essays, respectively. The first thesis accepted was in 1977. Because the overall study included 36 years, this period was classified into three parts, each part comprising 12 years ( Figure 1 ). The number of topics in gynecology and obstetrics were 367 and 336, respectively ( Tables 1 and ​ and2). 2 ). Infertility was the most common topic in gynecology. Topics were updated from basic investigations like hysterosalpingography to different techniques of artificial insemination (AI) and IVF-ET. Research also evaluated different parameters of ovarian reserve and its relation to the outcome of IVF. As regards to polycystic ovarian disease (PCOD), research included diagnostic, biochemical, ultrasonographic criteria, and role of insulin sensitizers. Research in gynecologic oncology included recent aspects of diagnosis and treatment of different gynecologic tumors. Role of laparoscopy in characterization of adnexal masses, management of large ovarian cysts, hysterectomy, and pelvic lymphadenectomy in early cancer cervix are examples. Research also covered new tumor markers like human epididymis protein 4. The relation between human papilloma virus and genital tract malignancy was also studied. Ultrasound and color Doppler were used to predict depth of myometrial invasion in cancer endometrium.

Preeclampsia was the first topic in obstetrics, and research included prediction, pathogenesis, clinical criteria, and laboratory investigations. Use of ultrasound and color Doppler was extensive.

Theses including skills were 183, which represented 31.8% of theses and 26% of all research. As shown in Figure 1 , the numbers increased from 8 of 163 (4.4%) in the period 1979–1989 to 91 of 274 (33.2%) in the period 1989–2000, and then slightly decreased to 84 of 266 (31.2%) in the period 2001–2012. Ultrasonography was the most common topic (99 theses, 54.1%) followed by laparoscopy (30, 16.4%), hysteroscopy (25, 13.7%), AI, IVF-ET, and related techniques (16, 8.7%), and lastly colposcopy (13 theses, 7.1%) ( Figure 2 ). Ultrasonography and color Doppler research included prediction, pathogenesis of preeclampsia as well as prediction, diagnosis, and monitoring of fetal growth restrictions. In preterm labor, ultrasonography was used in the prediction of preterm labor by measuring cervical length. Theses also included diagnostic criteria of placenta accreta. In abnormal uterine bleeding, ultrasound was used in evaluation of uterine cavity. In addition hydrosonography was also used in comparison to hysteroscopy. Techniques involved transabdominal, transvaginal, and transperineal examinations.

Research in laparoscopy included infertility, ovarian drilling for PCOD, and different techniques of laparoscopic hysterectomy. In hysteroscopy, theses included comparison between hysteroscopy and hysterosalpingography, as well as comparison between CO 2 and normal saline as distending medium. Hysteroscopy also used before and after failed IVF-ET. Operative hysteroscopy included monopolar and bipolar resectoscopic surgery. Research also included salpingoscopy and fallopian tube catheterizations.

Supervisions from another department in our faculty were present in 297 theses ( Table 3 ). Supervision from other universities (national experts) was noted in 10 theses, and international experts in 7 theses. The total supervisions beyond our department were present in 314 cases, which represent 54.7% of theses ( Tables 3 – 5 ).

This study shows that scientific research included in theses and essays for the master’s and doctoral degrees was comprehensive. Important topics in the specialty of obstetrics and gynecology were covered. Infertility was the most common topic in gynecology, followed by gynecologic oncology. Both topics represented about half of all theses and essays in gynecology. Preeclampsia was the most common obstetrics topic (18.8%), followed by issues of high-risk pregnancy, fetal growth restriction, and fetal well-being (11.6%). The study also shows that scientific research was updated. This was more evident regarding infertility research. We observed that not only the number of researches in infertility was steadily increased but also the topics were updated. Issues of AI, IVF-ET and related techniques, ovarian reserve, and role of endoscopy were covered. Recent research areas on PCOD were also studied. Actually, infertility became a well-established subspecialty mainly due to marked advancement in IVF-ET and related techniques in last three decades. 6 , 7

We observed that there was a marked increase in the number of researches after establishment of specialized units of ultrasonography, endoscopy and IVF-ET ( Figure 1 ). This was more evident for research using ultrasound. As shown in Figure 2 , theses including ultrasound jumped from five in the period 1977–1988 to 49 in the period 1989–2000. Actually, advances in technology have expanded the field of obstetrics and gynecology and made training and developing skills in different subspecialties a great challenge. 8 , 9 It seems that candidates preferred scientific research that enabled them to gain skills, and ultrasonography represented the first choice. Ultrasonography is relatively easy to learn, with great application in clinical practice. 10 Similar increase, but to a lesser extent, occurred in research in field of laparoscopy and hysteroscopy. The least number of researches were observed in the field of colposcopy. This could be explained by the absence of any national program for screening of cervical cancer. Actually, cervical cancer is not a great health problem in Egypt. 11 Similar findings were observed regarding research in gynecologic oncology which was increased by 1.5 times in the last 12 years with the establishment of the Gynecologic Oncology Unit.

Multidisciplinary collaboration in research is important. It improves research quality, increases research output, and also improves the communication skills of the researchers. 12 , 13 In the present study, more than half of the theses had supervision from another department, either academic or clinical. However, as shown in Table 3 , there was a reduction in the number of supervisions from 141 to 45 (61.7%) in the last 12 years. This may be explained by a decreased interest in academic research such as anatomy, physiology, and experimental work. The interest in ultrasound, laparoscopy, and hysteroscopy is clear, even though research in some clinically important areas like preeclampsia was also decreased ( Tables 1 and ​ and2). 2 ). Therefore, proper training of residents during their rotation in these subspecialties may help to revive the lost interest in clinically important areas.

The strength of this study is that it reviewed all master and doctoral theses and essays conducted through 36 years. This long period makes the results about changes in trends in research reliable and makes study beneficial for planning of scientific research and training of residents in obstetrics and gynecology. However, the study did not include other research published by staff members for promotion to the posts of professors. Actually, some staff members research are based on these master and doctoral theses. Therefore the study is typically concerned with research done by masters and doctoral candidates but still represents the scientific research in obstetrics and gynecology.

In conclusion, scientific research of master’s and doctoral theses and essays was comprehensive, updated, and had some autonomy independent of plans. Background: There have been significant gains in smoking cessation among pregnant women, but the rate of postpartum relapse remains high. Smoking is harmful to both the mother and child's health and women need to be educated about the risk of relapse and effective strategies to remain smoke free. The high rate of early postpartum relapse suggests that interventions must take place soon after delivery. Perinatal nurses are in the ideal position to begin the introduction of relapse prevention strategies, but many do not feel confident in providing these interventions. Despite evidence that nurses can be effective in providing tobacco counseling, few nurses actually follow through with recommended guidelines and assist and arrange follow up care.

Methods: A multi-site, interventional study was conducted using a pre-test/post-test design. A total of 162 perinatal nurses from four hospitals participated in a smoking cessation and relapse prevention counseling education program. Participants completed pre, post and one month follow up tests assessing perceived knowledge, attitude, self-efficacy, and behavior toward smoking cessation and relapse prevention counseling in the postpartum period. Descriptive statistics were used to characterize respondents; one way repeated ANOVAs were used to evaluate differences in scores on attitude, self-efficacy, knowledge and behavior.

Results: There was a significant increase in scores on knowledge, self-efficacy and behavior from pretest to follow up test. Although quitline referral scores increased from pre to follow-up test, the scores were very low. There were no significant differences in scores related to participant age, years of experience or level of education. OB nurses had significantly higher scores than neonatal nurses on all constructs.

Conclusions/Implications: Results indicate that a brief educational program is effective in increasing perinatal nurses' tobacco counseling knowledge, self-efficacy and behavior. Specific tobacco counseling educational programs for neonatal nurses need to be developed. Interventions are needed to increase nurses' quitline referrals for postpartum women at risk of smoking relapse.

Postpartum hemorrhage is the leading cause of maternal mortality and morbidity globally and affects women with a gestational age of 20 weeks and above. To determine whether the utilization of synthetic Pitocin in the intra-partum period has any association to postpartum hemorrhage, the primary investigator encouraged clinicians to continuously utilize the Triton OR to quantify the blood loss for 24 hours after vaginal delivery. The project also entailed chart review of 30 women who labored and delivered at a local hospital in the State of Georgia. One group was given synthetic Pitocin to induce and or augment their labor while the other group did not. Two clinical questions guided the project. What is the difference in the amount of blood loss within 24 hours of vaginal delivery between women who had their labor induced and or augmented with synthetic Pitocin compared to those who spontaneously labored and delivered? Is there any association between the utilization of synthetic Pitocin for the induction and or augmentation of labor, and post-partum hemorrhage within 24 hours of vaginal delivery? Jean Watson theory of human caring guided guide the project. A quantitative comparative design was utilized for the project. The mean blood loss for those women who received synthetic Pitocin was 279.20 milliliters, while that of women who did not receive synthetic Pitocin was 293.67 milliliters; hence there was no association between the utilization of synthetic Pitocin in the intra-partum period and postpartum hemorrhage ( p >.05). Conducting the project on a larger scale is paramount in determining whether there is any association between synthetic Pitocin utilization in the intra-partum period and postpartum hemorrhage.

The American Academy of Pediatrics (2012); the American College of Obstetricians and Gynecologists (2016); and the Association of Women’s Health, Obstetric, and Neonatal Nurses (2015) endorses breastfeeding exclusively for six months, followed by continuation with complementary foods through age one year to achieve the most personal and public health benefits. However, while most mothers initiate breastfeeding, the majority do not meet their goals due to lack of support (CDC, 2016).

The Affordable Care Act mandates coverage for breastfeeding support, but federal, state, and insurance policies are not conducive to making such benefits accessible.

Electronic registration and medical record documentation allow for improved tracking and decreased risk to the facility.

This paper outlines the development and implementation of administrative practices in an existing Registered Nurse/Internationally Board-Certified Lactation Consultant (RN-IBCLC) led outpatient lactation clinic at a Baby Friendly (2012) designated facility with goals of increasing access to lactation support postpartum, improving patient outcomes, and realization of financial viability. Expected outcomes included identification of best practices and implementation strategies for the following: a. provider collaboration including outreach efforts to trigger referrals. b. number of dedicated hours per week to offer services. c. cost/benefit analysis. d. registration and documentation process. e. model of care. f. funding source.

Tests of change through plan-do-study-act (PDSA) cycles were based on the outcome of a gap analysis. Modifications to implementation were made dependent on PDSA cycle findings until optimized RN-IBCLC-led outpatient lactation clinic administrative practices for the facility and processes became streamlined.

Despite limited literature to guide administrative practices, the project met the aims of patient registration and electronic health record (EHR) documentation. However, extensive policy and systems barriers existed that prevented success in identifying a workable revenue stream despite the efforts of this investigator in collaboration with parent system-level managed care, finance, and compliance departments.

The solution agreed upon to provide sustainability to the program was to utilize outpatient lactation visits as a community benefit to quantify lost revenue of services provided and aid the hospital. Even though the lack of revenue equaled lack of department growth, the community benefit option added a layer of stability to the program as it stands.

This project provides a model for other organizations examining best practices in administrative and funding options for IBCLC-led outpatient lactation clinics. Thereby; creating sustainable breastfeeding support leading to improved health of mothers, babies, and the community.

A nutritional method commonly used to deliver feedings to premature infants is the use of a gavage tube. To measure for any undigested breastmilk or formula, a gastric aspirate is checked prior to the next feeding. There is a gap in practice as to what to do if these aspirates signify feeding intolerance. The project question centered on identifying evidence-based guidelines in the literature that would help to define best practices related to feeding intolerance of gavage-fed infants. The Johns Hopkins Nursing Evidence-Based Practice model and the Appraisal of Guidelines Research and Evaluation provided the frameworks for gathering and evaluating evidence as well as the process used in forming the practice guideline. The primary methods employed were a team approach that included a Neonatal Intensive Care Unit (NICU) Project Team and NICU expert opinion along with a literature review conducted by the doctor of nursing practice student. The NICU Project Team collected the NICU experts’ input via surveys they developed and distributed as well as e-mails to authors identified from the literature review. The surveys yielded a 76% response rate from the registered nurses and a 59% response rate from the medical providers. All data collected were shared and descriptive statistics were used to evaluate the data. One of the central research findings was that gastric aspirates should no longer be routinely obtained on stable infants and, if used in evaluating feeding intolerance, they must be used in combination with other indicators. An enteral feeding guideline was developed to reflect this finding that can be shared with other NICUs and nurseries in the United States and globally to decrease the morbidity and mortality of neonates.

Traumatic birth is a term used to describe a wide-range of negative physical and mental birthing events and outcomes. An estimated one-third of women perceive mental trauma during birth. Aside from deterioration of maternal mental health, the effects of perceived traumatic birth can be observed in the entire family. Although research shows the relationship of traumatic birth to mental health, the majority has focused on European or European-American women. The profound psychological impact of birth trauma has been passed off as common to all women without considering culture, race, or context. Little is known about birth trauma experiences of Mexican immigrant women. Since Mexican immigrants comprise the largest ethnic-minority group in the United States attention to perceived traumatic birth on their health status is clear.

The purpose of this dissertation research was to describe and understand the impact of a perceived traumatic birth, the range of responses produced, and the life course context in which it occurs in Latina immigrants from Mexico.

Although recruitment took place in a low-income community-based health center in Fresno, California, most interviews were conducted in the participants’ homes. In addition to field observation and photography, twenty-one interviews were conducted with seven Mexican immigrant mothers who provided first-person accounts of their perceived traumatic births and the life course context in which it occurred. Facilitated by a certified Spanish interpreter, all interviews were recorded, transcribed verbatim and analyzed via an iterative grounded theory process.

The results suggest that, in addition to the birth event, for these Mexican immigrant mothers, the process contributing to perceived traumatic birth is embedded in the profound adversity of their unique life context. The burden of crossing a militarized border, the fear of deportation, and lack of trust, mandates a self-protective silence that obscures both the adversity they endure and the resulting maladaptive psychological responses. Consequently, for Mexican immigrant mothers, undocumented status, adversity, mistrust, and the self-mandated silence that surrounds it, may be major factors affecting not only their perceived traumatic births, but their overall health. Findings inform further research, practice, and policy related to the unique needs of Mexican immigrant mothers.

Every patient has a right to be treated with dignity, respect and high quality pain management (Olivier, et al., 2012). Pain continues to be inadequately treated by healthcare providers (Zuccaro, et al., 2012). The purpose of this DNP project was to determine if perinatal nurses’ intent to medicate pregnant women with chronic pain was affected by knowledge of pain, attitude, or demographics. This will explore the theoretical application of Ajzen’s theory of planned behavior which suggests that attitude correlates with intention to act.

A quantitative, cross-sectional correlation study used a pencil and paper survey to measure knowledge about pain, attitudes, and intent to medicate pregnant women with chronic pain. One hundred perinatal nurses who worked in labor and delivery, mother baby, or the neonatal intensive care unit from four hospitals in northern New Jersey participated in the study. Each hospital provided a different level of perinatal care from community basic, to intermediate, intensive, and regional perinatal center.

Increased levels of education positively impact perinatal nurses’ knowledge of pain, attitude, and intent to medicate pregnant women with chronic pain. The perinatal nurse’s intent to medicate was not statistically correlated to age, years of nursing experience, or level of perinatal care. The perinatal nurse’s intent to medicate a pregnant woman with chronic pain is positively correlated to increased knowledge of pain ( r (100) = 0.463, p ≤ 0.001). Attitude scores were positively correlated with an increased intent to medicate a pregnant woman with chronic pain ( r (100) = 0.583, p ≤ 0.001).

Adolescent pregnancy and parenthood have been identified as national public health problems and are topics of intense debate in the United States because of their impact on maternal and child health and on the social and economic well-being of the nation. While many types of parenting programs are available to the adolescent mother, not one approach has emerged as the most effective to decrease subsequent pregnancies and to improve mother and child health. Using Husserl's Phenomenology as a philosophical underpinning, what it means to be a pregnant adolescent living in a group home was explored. Data were collected from volunteers who had lived in two group homes in a suburban county in the Mid-Atlantic area of the U.S. In-depth, one-on-one interviews using a semi-structured guide were completed with eight participants. Giorgi's steps for analysis of the verbatim transcripts were used to develop essences and the essential structure about the phenomenon understudy. The participants' ages when they lived in the group home ranged from 13-17 years; the time they lived in the group home was between one to two years and some adolescents, after delivery, lived in the group home from six months to two years. Three themes emerged, each with subthemes, describing the participants' experiences: The environment with its rules and structure was experienced as either supportive or not in day-to-day living; Balancing adolescent expectations and needs impacted pregnancy and parenting; and Defining motherhood focused on the basics and was influenced by the participants' own mothering . The study's fmdings were compared and contrasted with well-known nursing, individual development, and parenting theories and relevant research findings. Nurses who care for adolescents throughout their pregnancy and the postpartum period are uniquely positioned to influence their development as mothers.

Women and men’s preferences for delivery services in rural Ethiopia Nancy Beam Aims: This study aims to determine the combination of facility-based delivery care attributes preferred by women and men; if gender differences exist in attribute preferences; and key demographic factors associated with attribute preferences.

Background: Despite programs to promote facility-based delivery, which has been shown to decrease maternal and neonatal mortality, 80% of women in rural Ethiopia deliver at home without a skilled birth attendant.

A review of the Ethiopian literature on factors associated with delivery location revealed several weaknesses in research methods that need to be addressed. First, research participants were almost exclusively women, although male partners often make decisions about delivery location. Second, most quantitative study designs are similar in content to the Ethiopian Demographic Health Survey, limiting the generation of new knowledge. Third, cultural practices identified in qualitative studies as barriers to facility-based delivery have not been included in quantitative studies. This study addressed these weaknesses by using discrete choice experiment methodology to elicit preferences for delivery service attributes, including support persons in the delivery room, staff training and attitude, cost, distance and transportation availability.

Methods: A cross-sectional, discrete choice experiment was conducted in 109 randomly selected households in rural Ethiopia in September-October 2015. Women, who were pregnant or who had a child < 2 years old, and their male partners were interviewed. After completing a demographic questionnaire, male and female respondents were asked separately to choose between facility-based scenarios that reflected various attributes for delivering their next baby. Data were analyzed using a multilevel mixed-effects logistic regression model.

Results: Both women and men preferred health facilities where medications and supplies were available, a support person was allowed in the delivery room, cost was low, and doctors performed the delivery. Women also valued free ambulance service, while men favored nearby facilities with friendly providers. Men are disproportionately involved in making household decisions, including decisions about whether their wives seek health care. Yet, men are often unaware of their partners’ prenatal care attendance.

Implications: The Ethiopian government and health facilities could increase facility births in rural areas by responding to families’ delivery service preferences.

Women involved with the criminal justice system face higher rates of unintended pregnancy than the general population, yet less than one-third use a consistent method of contraception. One study found that among women leaving detention, 43% had conceived within one year of release. Pregnancies that do occur are often high risk and result in poor outcomes for both mother and child. Lack of focus on family planning needs post-incarceration are due to competing factors women face related to daily survival and the added demands of meeting the requirements of probation.

This study examined the influences of pregnancy attitude, reproductive autonomy, personal factors and prior related behaviors on the use of effective birth control among women on probation living in southern New Mexico including the U.S.- Mexico border region. Using a quantitative correlational design framed by Pender's Health Promotion Model, 52 women were surveyed at five different Adult Probation and Parole Offices in two U.S.-Mexico border counties and two additional counties in southern New Mexico. Data analysis was conducted using descriptive statistics and logistic regression (single, multivariate, and hierarchical) to answer the following questions about women on probation: Do personal characteristics (contraceptive self-efficacy, birth control method prior to incarceration, age, ethnicity, and parity) significantly predict current birth control method? Which combination of personal characteristics (ethnicity, contraceptive self-efficacy, age, and parity) best predicts higher negative pregnancy attitudes and higher reproductive autonomy? Do pregnancy attitude and reproductive autonomy significantly predict current birth control method.

Results indicated a significant relationship between increased levels of reproductive autonomy (an interpersonal influence) and effective use of birth control among women on probation. While statistical significance was attained for two additional variables, contraceptive self-efficacy and prior birth control use, the results were not decisive due to widened confidence intervals. Use of a hierarchical logistic regression was effective for entering predictor variables into the regression based upon Pender's theoretical framework as a guide. Implications for nursing research, education, and practice were discussed. Future studies using larger sample sizes and additional settings would increase validity and generalizability.

This study was to determine if any difference exists in the rates of cesarean birth, emergency room visits and preterm birth in African-American women who participated in CenteringPregnancy ® group prenatal care in comparison with those in traditional prenatal care. Prenatal care under this model emphasizes risk assessment, health promotion, social support and education in a group setting of between eight and twelve patients. The effectiveness of CenteringPregnancy has had favorable clinical and behavioral outcomes, as well as high patient and provider satisfaction. Since few studies have researched outcome variables, this study was to answer if CenteringPregnancy prenatal care is related to improved birth outcomes than traditional prenatal care. Leininger’s Transcultural Nursing Theory guided this study to provide a comprehensive and culturally sensitive nursing approach in caring for pregnant African-American women. The population was a convenience sample of African-American women between the ages of 15 and 38 years of age. A retrospective chart review was utilized for data collection and a total of 61 CenteringPregnancy charts and 62 traditional prenatal care charts were reviewed looking at the rates of preterm births, cesarean births and emergency room utilization between the two groups of women. A bivariate statistical analysis using the t -Test was utilized to describe any differences between the patients in the two different types of prenatal care and a chi-square was used to analyze any difference in frequency of preterm births and cesarean births between the two types of prenatal care. The results indicated that African-American women receiving CenteringPregnancy prenatal care had fewer preterm births than African-American women in traditional prenatal care (1.6% vs 11.3%). However, there was no evidence found that African-American women receiving CenteringPregnancy prenatal care had less cesarean births or utilized the emergency room during their pregnancies less frequently.

Recent changes in the ethnic composition of the population of the United States pose great challenges for healthcare institutions and healthcare providers. In recent years, policy statements on nursing education indicated that nursing educators recognized the need to increase the cultural caring of the nursing workforce (Swanson, 2012). Salimbene (2014) discussed the importance of considering clients’ culture as an integral part of assessing their healthcare needs and planning culturally appropriate nursing care to meet those needs. Currently, there is a paucity of information regarding patients’ perceptions of culturally competent care. The purpose of this qualitative study was to identify culturally sensitive caring behaviors of professional nurses from the perspective of Hispanic American maternity patients two to four weeks post-discharge in a WIC program in rural Georgia. Utilization of a phenomenological research design included interviews with 15 Hispanic American women. Data analysis was conducted using phenomenological analysis methods with the aid of the software program Nvivo 11. Themes that were identified were: Better Future, Better Medical Care, Treatment of Patients, Customs and Practices, and Meaning of Care. This information may aid in creating a culturally competent maternity care curriculum.

The National Database of Nurse Quality Indicators (NDNQI) contains many nurse sensitive indicators for benchmarking adult medical/surgical patient outcomes but no perinatal nurse sensitive indicators. Project objectives were to form a collaborative, identify a perinatal nurse sensitive indicator, metrics that would contribute to its success and methods to measure and report them. The researcher developed a theoretical framework and three phase process to support the formation of the collaborative based on empowerment theory, collective impact and the Plan, Do, Study, Act cycle. Seven Midwest magnet organizations participated and identified breastfeeding as the indicator, along with three metrics that promote it: baby to breast in the first 60 minutes, non-separation of mom and baby, and skin-to-skin contact during the recovery period. Methods to document, measure and report metrics were developed and a database was selected for data comparison. While all objectives were met, only three organizations remained at project completion.

Background: Each year, perinatal loss affects one million women and their families (National Vital Statistic Reports, 2011). Perinatal loss is the death of a fetus/infant during pregnancy or soon thereafter (Koppmans, Wilson, Cacciatore & Flenady, 2013). There are several different types of perinatal loss, including miscarriage, stillbirth, and neonatal death. Bereavement education related to miscarriage will be the main focus of this project. Miscarriage, also known as spontaneous abortion, is the body's natural termination of a pregnancy before 20 weeks gestation (Geller, Psaros, & Kornfield, 2010).

Objective: The clinical problem to be addressed by this project is the lack of a system of evidence based bereavement education for women who experience miscarriage and subsequent treatment when they are admitted to the hospital through the Pre-Admission Unit (PAU)at WVU Healthcare. The project plan involves the development, implementation, and evaluation of an evidence based bereavement education system for women who are treated in the PAU prior to surgical intervention for a miscarriage. This intervention has been designed to address knowledge, skills, and attitudes of staff in the PAU related to providing bereavement support for women who experience a miscarriage.

Methods: A computerized literature search of CINAHL, PUBMED, National Guideline Clearinghouse, and the Cochrane Library data bases were performed using the key words miscarriage, spontaneous abortion, emotional experience, grief, education of providers, and bereavement education. Search limits were set which included the dates of 2002–2013, peer reviewed, full text, and English language. Selection criteria included the provision of grief interventions to women and their families who have suffered a perinatal loss and staff who cared for women who suffered a miscarriage. These searches produced 58 hits. After reviewing abstracts and article fourteen articles were chosen for the review. There were five systematic reviews, two randomized controlled trial (RCT) studies, six descriptive qualitative studies, and one non-experimental, correlational study.

Results: Strong evidence existed that grief education could assist healthcare providers to be able to provide evidence based interventions to women and families with a miscarriage. This project validated the literature by indicating that healthcare providers that were given bereavement education were much more comfortable presenting this education to women who suffered a miscarriage and required surgical intervention. The increase in knowledge of the healthcare providers was established with a pre-test, intervention of providing bereavement education, and a post-test. A t-test was used to compare means of the pre and post-tests and conclude that there was statistical significance in the mean scores of the pre and post-tests.

Obstetric hemorrhage is one of the most common causes of maternal morbidity and mortality. The measurement of quantitative blood loss (QBL) at delivery prevents clinicians from failing to recognize hemorrhage in healthy obstetric patients who initially compensate for excessive blood loss. The purpose of this project was to improve the compliance of labor and delivery nurses in a community hospital with consistent QBL measurement. Key theories that formed the basis for the project were Lewin’s theory of planned change and homeostasis. The project question addressed was: Is the use of weekly scorecards to provide feedback to nurses with both blinded individual data and aggregate unit data associated with an increase in the percent of patients with blood loss at delivery documented as a QBL measurement over a 12-week period of time? A blinded scorecard of the percent of deliveries attended by each nurse that had QBL documented and an aggregate run chart of the percent of all deliveries with QBL documented were posted in the unit weekly. The postings included discussions of means to enhance facilitators of and decrease barriers to QBL measurement. Over 12 weeks, the percent of deliveries with QBL documented increased from 22.7% to 80.0%. This result is consistent with previous reports that clear and objective feedback from scorecards is associated with improvement in performance. Scorecard feedback may be explored to determine if it is associated with improvement of other nursing practices. This project has implications for positive social change as it may contribute to a reduction in preventable maternal deaths. Decreasing maternal morbidity and mortality supports the health of women in a population and influences the health of the next generation.
