If you need to secure a certificate from the Philippine Statistics Authority (PSA) but do not have the time to get it yourself, you can authorize someone else to get it in your place. To do that, you must write a letter addressed to PSA stating that you allow that person to secure a PSA certificate. This article will guide you on  how to write an authorization letter for PSA . The Philippine Statistics Authority issues four certificates:

Who is allowed to request documents from PSA?

Following Republic Act No. 10173, popularly known as the “Data Privacy Act of 2012,” the Philippine government has the policy to preserve each person’s fundamental right to privacy.

According to PSA, the following entities are allowed to request a copy of the issuance of a birth, death, or marriage certificate, or a Certificate of No Marriage (CENOMAR) / Advisory on Marriages (AOM):

  • The owner of the document must be of legal age (18 years or older) or the owner’s authorized representative.
  • Legal partner
  • Children of the document’s owner who are of legal age
  • The court-appointed guardian
  • The surviving grandparents
  • The oldest sibling, over 21, unless they are unfit or disqualified.
  • actual custodian of the kid, at least 21 years old, unless unfit or ineligible.
  • an institution with legal custody of the child and approval from
  • The court or the appropriate government authority by way of a subpoena duces tecum and ad testificandum
  • government agencies acting in accordance with their mandates and in accordance with National Privacy Commission Circular No. 16-02
  • If the document owner has passed away, the closest relative may take possession of the document as long as that relative signs and provides an Affidavit of Kinship certifying that they are the document owner’s closest living relative.

Who is an Authorized Representative?

A person of legal age who has been expressly authorized in writing (by an Authorization Letter or Special Power of Attorney) by the document owner to request a copy of the document owner’s birth certificate, marriage certificate, certificate of no marriage (CENOMAR), or advisory on marriage (AOM) from the PSA.

What should be the contents of the Authorization Letter or the Special Power of Attorney?

  • Names of the approving individual(s)
  • A specific purpose or use in getting the requested document or authentication
  • Type of document (certificate of birth, marriage, or CENOMAR/AOM)
  • The number of copies
  • Birth certificate: the owner’s full name, birth date, birthplace, and parents’ names
  • Full names of the husband and wife, as well as the wedding date and location, on the marriage certificate
  • The same information is on the CENOMAR/AOM as on the birth certificate.

1. Address your letter to PSA. (Add “To Whom It May Concern”)

2. Specify your full name.

3. State that you are giving your representative authority to acquire a PSA certificate.

4. Make a list of all the certificates you have authorized your representative to obtain.

5. Specify the purpose and application of the aforementioned certificate(s).

Other Requirements

  • The requester’s legitimate identification (ID) or identity document
  • a photocopy of the applicant’s legitimate ID or identity document.
  • original and photocopy of the document owner’s legitimate ID or identity document.
  • Original copy of the duly signed authorization letter or SPA, with the document owner’s signature matching the signature on the accompanying legitimate ID or identity document. If the SPA serves multiple purposes, a copy of it should be included.

  • Senior Citizen’s ID card issued by the Office of the Senior Citizen’s Affairs and /or Local Government Unit
  • OFW ID issued by the Department of Labor and Employment
  • Overseas Workers Welfare Administration ID
  • Seaman’s / Seawoman’s Book issued by the Maritime Industry Authority
  • Diplomat/Consular ID issued by the Philippine Embassy
  • NBI / Police Clearance
  • DSWD Certification / 4Ps ID
  • A person with Disability ID issued by the National Council on Disability or its regional counterpart, Office of the Mayor, Office of the Punong Barangay, DSWD Office, and other participating organizations with Memorandum of Agreement with the Department of Health
  • IDs issued by National Government agencies including Government Owned and Controlled Corporations
  • TIN Card issued by the Bureau of Internal Revenue
  • School / Student ID for currently-enrolled students, 18 years old or above issued by reputable schools/colleges/universities recognized by DepEd or CHEd and signed by the principal or head of the academic institution
  • Company IDs issued by private entities or institutions registered with, supervised, or regulated by the Bangko Sentral ng Pilipinas, Securities, and Exchange Commission, or Insurance Commission
  • Barangay ID / Certification with picture and signature
  • Alien Certification of Registration (ACR I-card);
  • Immigrant Certificate of Registration;
  • Special Resident Retiree Visa issued by the Bureau of Immigration through the Philippine Retirement Authority

In order to be accepted as valid, an identification card or identity document must have a clear photo of the holder, as well as the holder’s full name and signature.

  • Fill out delivery details. If you aren’t available to receive the document, you may assign authorized representatives to receive the document for you, as long as they have an ID and a letter of authorization.

Fees and payment method

After you complete your online application, you will be issued a Batch Request Number and, for each request, a Request Reference Number. When paying for all requests in the batch, specify the Batch Request Number. When paying for a particular request only, specify the Request Reference Number.

Fees are inclusive of processing, delivery, and government taxes.

How much are the fees to request birth/marriage/death certificates or CENOMAR?

The fees and the available payment channels depend on whether your delivery address is within the Philippines or outside the Philippines. Listed below are the base fees for each document.

The  Philippine Statistics Authority   (Filipino: Pangasiwaan ng Estadistika ng Pilipinas) , abbreviated as PSA, is the central statistical authority of the Philippine government that collects, compiles, analyzes, and publishes statistical information on economic, social, demographic, and political affairs and general affairs of the people of the Philippines and enforces the civil registration functions in the country. It is an attached agency of the National Economic and Development Authority (NEDA) for purposes of policy coordination. The PSA comprises the PSA Board and offices on sectoral statistics, censuses, technical coordination, civil registration, the Philippine registry office, central support, and field statistical services.

The National Statistician, who is appointed by the President of the Philippines from a list of nominees submitted by a Special Committee and endorsed by the PSA Board Chairperson, is the head of the PSA and has a rank equivalent to an Undersecretary. Aside from directing and supervising the general administration of the PSA, the National Statistician provides overall direction in the implementation of the Civil Registry Law and related issuances and exercise technical supervision over the civil registrars as Civil Registrar General.

Furthermore, the PSA shall primarily be responsible for the implementation of the objectives and provisions of R.A. 10625, R.A. 11055, and R.A. 11315. It shall plan, develop, prescribe, disseminate, and enforce policies, rules, and regulations, and coordinate government-wide programs governing the production of official statistics, general-purpose statistics, civil registration services, and inclusive identification systems. It shall primarily be responsible for all national censuses and surveys, sectoral statistics, community-based statistics, consolidation of selected administrative recording systems, and compilation of national accounts.

How to Apply

Applicants are required to submit the following:

  • Scanned copy of the signed APPLICATION LETTER stating the specific position title with salary grade (SG) level and the specific place of assignment  as posted. An applicant may apply for a maximum of two (2) positions; The application letter must be addressed to:
  • Scanned copy of duly accomplished and wet signed  PERSONAL DATA SHEET  (PDS)  with recent ID picture taken within three (3) months prior to submission of application (CSC Form No. 212, Revised 2017); duly subscribed and sworn to before an authorized administering officer (refer to Guide to Filling Out the Personal Data Sheet or CSC MC No. 16, s. 2017); Any inaccurate information that affects the qualification of the applicant to the position he/she is applying for, i.e., the accomplishment of Part III (Educational Background), Part IV (Civil Service Eligibility), Part V (Work Experience), and Part VII (Learning and Development (L&D) Interventions/Training Programs Attended), and absence of other substantial entries such as the signature of the applicant on each page and signature of the authorized person to administer the oath in page 4, which are not compliant with the said CSC MCs shall be grounds for exclusion from the recruitment process;
  • Scanned copy of signed  WORK EXPERIENCE SHEET  (CSC Form No. 212 – as Attachment to PDS);
  • Scanned copy of  AUTHENTICATED ELIGIBILITY  (CSC, RA 1080, PD 907, etc.) (if applicable);
  • Scanned copy of  TRANSCRIPT OF RECORDS  and  DIPLOMA ;
  • Scanned copy of  CERTIFICATE/S OF TRAINING  participated within the last five years relevant to the position applied for (if applicable);
  • Scanned copy of  CERTIFICATES OF WORK EXPERIENCE  relevant to the position (for applicants with work experience).

  • The PSA reserves the right to suspend or terminate the hiring process for qualified reasons or disqualify any and/or all applicants for failure to comply with any of the foregoing requirements and instructions without incurring liabilities against any of the applicant/s.
  • Applicants must present the original copy of the documents/attachments to the Secretariat before the interview. In case the interview is online or via video conference platform and it is difficult for the candidate to appear before the Secretariat, the presentation of original documents for verification of authenticities are subject to instruction of the Secretariat appropriate for the situation. The inability to present any of these documents will lead to automatic disqualification.
  • Applications with incomplete attachments and/or incomplete information or signature within a particular attachment will not be considered.
  • Interviews of pre-qualified applicants will be conducted at the PSA Central Office located at Eton Centris Complex, EDSA cor. Quezon Ave., Diliman, Quezon City

The Committee on COSWs may conduct an online interview or change the venue of the interview subject to one (1) day prior notice to the concerned applicants via email or mobile/cellular phone.

  • the owner’s parent/guardian or others who are legally granted parental authority
  • the owner’s spouse;
  • owner’s immediate descendants (of legal age)
  • authorize person of the owner

When the person on the certificate is a minor, only the parents or guardians are allowed to apply for and receive the document with a valid ID.

Authorization Letter or Special Power of Attorney (SPA)

This is only required when the applicant and the receiver is not the owner. It must indicate the purpose of the document, the type of document you are applying for, the number of copies, and other details. Try to make it as specific as possible. The authorization letter must be signed by the owner. Please submit a handwritten or printed copy instead of sending it through e-mail or instant messaging apps. The SPA must be notarized.

Located in almost every town and city in the Philippines, it is quite easy to find. If you have no access to a birth certificate online and are in no hurry, you can seek walk-in applications at the SM business center through the following steps.

  • Find a SM Business Center which is usually located in the SM supermall. Check this mall directory and enquire whether there is one in the nearest mall.
  • Obtain an application form with a valid ID in the civil registry section.
  • Fill out the form and make sure the information you provide is complete and accurate.
  • Hand in the completed form to the cashier and pay the application fees. You will receive a receipt and a claim stub which tells you when the birth certificate is ready. it usually takes 4-6 days.
  • Collect the document on the scheduled date with the ID, receipt, and claim stub. If you are not the owner, your ID and authorization letter are also required.

It is the fastest way to get a certificate, but you should make a PSA birth certificate online appointment first. Let’s see how to achieve it.

  • Visit the Civil Registration Service Appointment System . After reading the instructions and Privacy Notice, click the box in front of “I Agree”. Please note that minors cannot use this booking system.
  • Choose “Get copy/ies of civil registry document/s and certification/s” for the purpose of your appointment and choose a Civil Registration System (CRS) outlet.

  • Attend the appointment at the scheduled time with a pen, the original copy and photocopy of your valid ID and other supporting documents. It is suggested to arrive a few minutes in advance.
  • Wait in line and submit your application to the officer.
  • Pay at the cashier and keep the receipt.

HMRC will check what’s included in your PSA . They’ll contact you if there are any issues

If your request is authorised, you’ll get a confirmation email or letter. You’ll then be sent your PSA by post.

Write to HMRC Business Tax and Customs describing the expenses and benefits you want the PSA to cover.

Once HMRC have agreed on what can be included, they’ll send you 2 draft copies of form P626. Sign and return both copies. HMRC will authorise your request and send back a form - this is your PSA .

Tell HMRC what you owe

Once you have your PSA , you need to tell HMRC what you owe every tax year.

Complete and submit the online form . If you do not, HMRC will calculate the amount. You’ll be charged more if this happens.

You’ll need to report anything that cannot be included in your PSA separately using form P11D . You do not need to send a P11D if you’re paying employees’ expenses and benefits through your payroll.

What happens next

The agreement will continue until either you or HMRC cancels it or you need to change it . You do not need to renew the PSA each tax year.

How to Write a PSA (Public Service Announcement)

What is a psa (public service announcement).

A public service announcement (PSA) is a short, community-oriented message that radio stations air at no cost in order to fulfill their obligation to serve the public interest. PSAs are a cost-effective way for non-profit organizations to raise awareness about the benefits their organizations provide. Radio stations receive PSAs as audio files – typically recorded as :30 or :60-second messages. These files can be accompanied by scripts for announcers to read live on air. Through appropriate distribution, they can be targeted to preferred demographics and markets.

Radio stations have a vested interest in serving their communities, and they really do want to help promote your non-profit message. However, depending on the time of year and the markets you want to reach, there might be dozens of other organizations competing for airtime.

Here is our list of the top five things you can implement as you conceive, write and produce your public service announcement to ensure your message reaches and resonates with the right audience.

How to Write a PSA (Public Service Announcement)

1. Be Authentic

PSAs must be linked to a 501(c)(3) tax-exempt, non-profit organization with local or national recognition. One way to ensure that your public service announcement gets the attention of radio station public affairs directors is to include an appeal on your organizational letterhead, signed by your communications director or your president. You’ll also want to direct their attention to your website and social media pages where they can find more information about your organization. If you are active and making an impact in your community, it should be visible online which will help increase your credibility and improve your odds of having your message air.

Every station has a different vetting process, but here’s a typical example from a station website of their requirements for submitting a PSA:

To Submit a PSA please email our Public Affairs Director with the following info:

  • 501(c)(3) Charity/NonProfit official name
  • Description of event, initiative or cause
  • Name of event, date, time and location, and a website if there is one.
  • Description of who your organization benefits, and how donations work (what portion of the proceeds go to the beneficiaries)
  • Your name, title and phone number

2. Keep It Simple – Easy – Quick

The key to getting a quick response is to make it simple for stations to access the PSA and reply. Provide them with an easy way to download the PSA and script from a website and an automated way to let you know if they’re using it. Years ago, we would record the PSAs onto CDs which we mailed to stations with an introductory letter, a copy of the script, and a self-addressed stamped postcard for them to respond. It worked fine then, but now our PSAs are submitted digitally and we get a much higher rate of participation and trackable response.

3. Carefully Select Format & Delivery

It’s best to create both a :30 second (65-90 words) and a :60 second (150-180 words) version of your public service announcement and to include the scripts of both versions, or alternate scripted versions. Some stations won’t air the produced piece but will instead enlist one of their hosts to read it on-air — we call that a “live reader.”  For an American Cancer Society PSA, we produced recorded versions of a 60-second and 30-second PSA, and a 15-second live reader script as well, after several stations requested something shorter than the 30-second option.

Keep in mind that emails with multiple attachments (like audio files) from unknown senders often get flagged by an email server as spam. To help prevent that from happening, craft an introductory email that includes a letter, the text of the PSA, and a link to a website or a Dropbox folder from where stations can easily download the audio and scripts.

Stations often schedule the rotation of their PSAs weeks or even months in advance. If you are promoting a time-sensitive event, you’ll want to begin reaching out to stations 6-8 weeks prior to the event date. 4. Follow-up & Track

Some radio stations may post a disclaimer about PSAs saying that “due to the volume received, we are not able to acknowledge receipt of your submission.” This is why it is essential that your campaign employs a well-planned effort to follow-up through phone calls and emails to as many stations as possible. This subsequent contact is also another opportunity to pitch the merits of your cause and persuade the station representative into airing your PSA.

Make it easy for stations to respond to your request for airing a PSA, even if the answer is “no, thank you.” We’ve had a lot of success with including a short response form on the same website page where stations download the audio and scripts. For a recent PSA about Opioid Abuse targeted to the State of Illinois, we used an online form, and followed up with emails and phone calls, resulting in nearly 50% of the responding stations using it as recorded, or doing live reads.

5. Measure Success – What is a good response?

The frequency and duration of how stations air public service announcements vary greatly. Some of the bigger stations might commit to daily airings for only a few weeks, depending on how many other PSAs they are running at the same time. However, if the content is “evergreen,” meaning that the PSA isn’t connected to a specific event in time and doesn’t contain information with an expiration date, it might be put into on-air rotation for several months. Expect a variety of air dates and time slots.

Some stations give priority to local nonprofits and it’s impossible to know how many other organizations are also competing for airtime, so a usage rate of 35-50% is considered a success. Anything above 50% is a home run!
